Enrollment with The Lone Star Veterans Health Initiative is designed to be simple, respectful of your time, and focused on protecting your personal information. Our goal is to get you connected to the right resources as quickly and safely as possible.
To enroll in our system, you’ll be asked to complete our intake process that helps us understand your needs and ensure you’re eligible for our programs. This information allows us to better match you with fitness, wellness, and support resources that fit you—not the other way around.

We only collect information that is necessary to serve you effectively, and all documentation will be kept private and secure and will not be kept online or accessible by any third parties. The Lone Star Veterans Health Initiative will NEVER sell your information.
We understand that many veterans are cautious about sharing personal information online, and that’s a good thing. Your data is handled with care and is never sold or shared outside of our trusted partner network used solely to deliver services.
If you have any concerns about document safety, privacy, or potential scams, you are always welcome to skip online submission entirely.

You can schedule an in-person appointment to meet with us directly and complete all enrollment paperwork face-to-face. Many veterans prefer this option, and we’re happy to accommodate it. Your comfort and confidence come first. Just use the "Contact Us" feature and send us an email with your request.

If you want to use our secure online enrollment, head to the top of the page an click on the Enroll Now! Button. You will be directed to our online enrollment application and intake form. Please complete the form to the best of your ability, and once we have received it we will reach out to finalize the enrollment.
